Procedure during Surgery

The procedure starts with the patient lying on a padded table. A coronary angioplasty is usually performed in a room called a catheterization laboratory. During a coronary angiography, a long, flexible, hollow plastic tube called a catheter (about the width of the lead in a pencil) is inserted into a blood vessel, either in your groin or arm. Atherosclerosis is a condition in which a material called plaque builds up on the inner walls of the arteries. This can happen in any artery, including the coronary arteries, which carry oxygen-rich blood to your heart. How Is Coronary Angioplasty Done ?

Before coronary angioplasty is done, your doctor will need to know whether your coronary arteries are blocked. If one or more of your arteries are blocked, your doctor will need to know where and how severe the blockages are.

To find out, your doctor will do an angiogram and take an x-ray picture of your arteries. During an angiogram, a small tube called a catheter with a balloon at the end is place into a large blood vessel in the groin (upper thigh) or arm. The catheter is then threaded to the coronary arteries. A small amount of dye is injected into the coronary arteries and an x-ray picture is taken.

This picture will show any blockages, how many, and where they’re located. Once your doctor has this information, the angioplasty can proceed. Your doctor will blow up (inflate) the balloon in the blockage and push the plaque outward against the artery wall. This opens the artery more and improves blood flow.

Figure A shows the deflated balloon catheter inserted into the narrowed coronary artery. In figure B, the balloon is inflated, compressing the plaque and restoring the size of the artery. Figure C shows the widened artery.

A small mesh tube called a stent is usually placed in the newly widened part of the artery. The stent holds up the artery and lowers the risk of the artery renarrowing. Stents are made of metal mesh and look like small springs.

Some stents, called drug-eluting stents, are coated with medicines that are slowly and continuously released into the artery. These medicines help prevent the artery from becoming blocked again from scar tissue that grows around the stent.

In some cases, plaque is removed during angioplasty. In a procedure called atherectomy a catheter with a rotating shaver on its tip is inserted into the artery to cut away plaque. Lasers also are used to dissolve or break up the plaque. These procedures are now rarely done because angioplasty gives better results for most patients.

What to Expect Before Coronary Angioplasty

Meeting With Your Doctor in India

A cardiologist (a doctor who treats people with heart conditions) performs coronary angioplasty at a hospital in India. If your angioplasty isn’t done as emergency treatment, you’ll meet with your cardiologist before the procedure. Your doctor will go over your medical history (including the medicines you take), do a physical exam, and talk about the procedure with you.

Your Indian doctor also will order some routine tests, including : -

* Blood tests
* An ECG (electrocardiogram)
* A chest x ray

When the procedure is scheduled, you will be advised : -

* When to start fasting (not eating or drinking) before the procedure. Often you have to stop eating or drinking by midnight the night before the procedure.
* What medicines you should and shouldn’t take on the day of the angioplasty.
* When to arrive at the hospital and where to go.

Even though angioplasty takes 1 to 2 hours, you will likely need to stay in the hospital overnight. In some cases, you will need to stay in the hospital longer. Your doctor may advise you not to drive for a certain amount of time after the procedure, so you may have to arrange for a ride home.

What to Expect During Coronary Angioplasty

Coronary angioplasty is performed in a special part of the hospital called the cardiac catheterization laboratory. The “cath lab” has special video screens and x-ray machines. Your doctor uses this equipment to see enlarged pictures of the blocked areas in your coronary arteries.

Preparation

In the cath lab, you will lie on a table. An intravenous (IV) line will be placed in your arm to give you fluids and medicines. The medicines will relax you and prevent blood clots from forming. These medicines may make you feel sleepy or as though you’re floating or numb.

To prepare for the procedure : -

* The area where the catheter will be inserted, usually the arm or groin (upper thigh), will be shaved.
* The shaved area will be cleaned to make it germ free and then numbed. The numbing medicine may sting as it’s going in.

Steps in Angioplasty

When you’re comfortable, the doctor will start the procedure. You will be awake but sleepy.

A small cut is made in your arm or groin into which a tube called a sheath is place. The doctor then threads a very thin guide wire through the artery in your arm or groin toward the area of the coronary artery that’s blocked.

Your doctor puts a long, thin, flexible tube called a catheter through the sheath and slides it over the guide wire and up to the heart. Your doctor moves the catheter into the coronary artery to the blockage. He or she takes out the guide wire once the catheter is in the right spot.

A small amount of dye may be injected through the catheter into the bloodstream to help show the blockage on x ray. This x-ray picture of the heart is called an angiogram. Next, your doctor slides a tube with a small deflated balloon inside it through the catheter and into the coronary artery where the blockage is. When the tube reaches the blockage, the balloon is inflated. The balloon pushes the plaque against the wall of the artery and widens it. This helps to increase the flow of blood to the heart. The balloon is then deflated.

Sometimes the balloon is inflated and deflated more than once to widen the artery. Afterward, the balloon and tube are removed. In some cases, plaque is removed during angioplasty. A catheter with a rotating shaver on its tip is inserted into the artery to cut away hard plaque. Lasers also may be used to dissolve or break up the plaque.

The doctor inserts a tube called a balloon catheter into a coronary artery narrowed by plaque. The balloon catheter compresses the plaque and widens the artery to restore blood flow.

If your doctor needs to place a stent (small mesh tube) in your artery, another tube with a balloon will be threaded through your artery. A stent is wrapped around the balloon. Your doctor will inflate the balloon, which will cause the stent to expand against the wall of the artery. The balloon is then deflated and pulled out of the artery with the tube. The stent stays in the artery.

After the angioplasty is done, your doctor pulls back the catheter and removes it and the sheath. The hole in the artery is either sealed with a special device, or pressure is place on it until the blood vessel seals.During angioplasty, strong antiplatelet medicines are given through the IV to prevent blood clots from forming in the artery or on the stent. These medicines help thin your blood. They’re usually started just before the angioplasty and may continue for 12-24 hours afterward.

Going Home and back to your Country after Coronary Angioplasty

Most people go home 1 to 2 days after the procedure and glide back to their country from India in a weeks time. When your doctor thinks you’re ready to leave the hospital,

you will get instructions to follow at home, including : -

* How much activity or exercise you can do.
* When you should follow up with your doctor.
* What you should look for daily when checking for signs of infection around the area where the tube was inserted. Signs of infection may include redness, swelling, or drainage.
* When you should call your doctor. For example, you may need to call if you have a fever or signs of infection, pain or bleeding where the catheter was inserted, or shortness of breath.

Your doctor will prescribe medicine to prevent blood clots from forming. Taking your medicine as directed is very vital. If a stent was inserted, the medicine reduces the risk that blood clots will form in the stent. Blood clots in the stent can block blood flow and cause a heart attack.

Revascularization patterns vary and are dependent largely on ACS subtype. STEMI patients in all countries showed greater use of thrombolytics compared to NSTEMI and UA patients. PCI represents a more aggressive method to reopen coronary vessels. PCI is widely used in all ACS patients, but is most common in STEMI, particularly in Germany and the US.

Time-to-PCI is very vital for myocardial salvage. Germany, followed by the US, showed the highest proportion of STEMI and NSTEMI patients achieving symptom onset to PCI time and door to PCI times in less than 12 hours. This is a function of Germany’s brilliant PCI infrastructure.

There is wide regional variation in the use of bare metal stents and drug-eluting stents. Germany and France show preference for BMS over DES in all ACS types. Additional data and newly available stents, have powered a shift in practice back towards use of DES, particularly in Italy, Spain, UK and the US.

What is Angioplasty?
Angioplasty can reopen clogged heart arteries during a heart attack or when plaque buildup is causing chest pain, shortness of breath or other symptoms. Coronary Angioplasty also referred to as percutaneous coronary intervention (PCI), is a medical procedure used to open clogged heart arteries.

Coronary angioplasty can improve some of the symptoms associated with blocked arteries, such as chest pain and shortness of breathe, or can be used during a heart attack to quickly open a blocked artery and minimize heart hurt.

Angioplasty (AN-je-o-plast-tee) involves temporarily inserting and expanding a tiny balloon at the site of your blockage to help widen a narrowed artery. Angioplasty is usually combined with implantation of a small metal coil called a stent in the clogged artery to help prop it open and decrease the chance of it narrowing again (restenosis).

Who are the Candidate for Angioplasty?

When medications or lifestyle changes aren’t enough to reduce the effects of artery blockages, or if you have a heart attack, worsening chest pain or other symptoms, your doctor might suggest Angioplasty. First you’ll have an imaging test called a coronary angiogram to determine if your blockages can be treated with Angioplasty.

You may be a excellent candidate for an Angioplasty if:

Your blockage is small
Your blockage can be reached by angioplasty
The artery affected isn’t the main vessel supplying blood to the left side of your heart
You don’t have heart failure

Who Performs an Angioplasty?

Angioplasty is performed by a heart specialist (cardiologist) and a team of specialized cardiovascular nurses and technicians, usually in a cardiac catheterization laboratory. Ideally, an Angioplasty will be done only at a medical institution with backup cardiac surgical facilities or access to rapid emergency transport services because of a small risk of complications that could require emergency heart surgery.

What happens during an Angioplasty?

Coronary Angioplasty isn’t considered surgery because it’s less invasive — your body isn’t cut opens except for a very small puncture in a blood vessel in the leg, arm or wrist through which a small, thin tube (called a catheter) is threaded and the procedure performed. The entire procedure can take 30 minutes to several hours.

Angioplasty is commonly performed through an artery in your groin (femoral artery). Less commonly, it may be done using an artery in your arm or wrist area.

Before Angioplasty procedure, the area is prepared with antiseptic solution and a sterile drape is placed over your body.
A local anesthetic is injected into your groin to numb the area.
Small electrode pads are placed on your chest to monitor your heart rate and rhythm during the procedure.
After numbing the incision area, a small cut is made, usually in your leg, to access an artery. Your doctor will then insert a thin guide wire into the artery and thread it through the artery from the incision area up to your blockage.
Once the guide wire reaches the blockage, a small, thin tube (catheter) is passed over the wire until it reaches the blockage. You might feel pressure in your groin while this is being done, but you shouldn’t feel sharp pain. You also won’t feel the catheter in your body.
A small amount of contrast agent, or dye, is injected through the catheter. This helps your doctor look at the blockage on X-ray images called angiograms.
A small balloon at the end of the catheter is inflated, widening the blocked artery. The balloon stays inflated for up to several minutes at the site of the blockage, stretching out the artery before it’s deflated and removed. Your doctor might inflate and deflate the balloon several times before it’s removed, stretching the artery a bit more each time to widen it.

Because the balloon temporarily blocks blood flow to part of your heart, it’s common to experience chest pain while it’s inflated. If you have several blockages, the procedure may be repeated at each site.

Stents provide added support

Once the artery is widened, a device called a stent is usually placed in the artery to act as scaffolding to help prevent it from re-narrowing after the Angioplasty. The stent looks like a very tiny coil of wire mesh. Stents can be coated with medication that’s slowly released to help prevent arteries from re-clogging. These coated stents are called drug-eluting stents, in contrast to non-coated versions, which are called “bare-metal” stents.

Here’s what happens during Stenting:

The stent is collapsed, placed around a balloon at the tip of the catheter and guided through the artery to the blockage.
At the blockage, the balloon is inflated and the spring-like stent expands and locks into place inside the artery.
The stent remains in the artery permanently to hold it open and improve blood flow to your heart.
Once the stent is in place, the balloon catheter is removed and more images (angiograms) are taken to see how well blood flows through your newly widened artery.
Finally, the guide catheter is removed and the procedure completed.

After your stent placement, you may need prolonged treatment with medications to reduce the chance of blood clots forming on the stent material.

The recovery period for Angioplasty

Post Angioplasty, you’ll probably remain hospitalized a day — in rare cases two or more days — while your heart is monitored and your vital signs are checked frequently. Your doctor will likely prescribe medications (anticoagulants) to prevent blood clots, relax your arteries and protect against coronary spasms.

Call your doctor’s office or hospital staff immediately if:

The catheter insertion site starts bleeding or swelling
You develop increasing pain or discomfort at the insertion site
You have signs of infection, such as redness, drainage or fever
There’s a change in temperature or color of the leg or arm that was used for the procedure
You feel faint or weak
You develop chest pain or shortness of breath

You should be able to return to work or your normal routine the week after Angioplasty.

Results after Angioplasty

For most people, coronary Angioplasty greatly increases blood flow through the previously blocked artery. Your chest pain should subside, and you may have a better ability to exercise.

Lifestyle modifications will help you maintain your excellent results, including:

Stopping smoking
Lowering your cholesterol levels
Maintaining a healthy weight
Controlling other conditions, such as diabetes and high blood pressure
Getting regular exercise

What is Balloon Valvuloplasty?

Balloon valvuloplasty is a procedure in which a narrowed heart valve is stretched open with a small balloon which is inserted and inflated to stretch and open a narrowed (stenosed) heart valve.

Why is Valvuloplasty performed?

There are four valves in the heart- aortic valve, pulmonary valve, mitral valve, and tricuspid valve. These valves are located at the exit of each of the four chambers of the heart and these valves open and close to regulate the blood flow from one chamber to the next and thus are very vital for the efficient functioning of the heart. When these valves become narrow (stenosis) due to hardening and calcium deposits, Valvuloplasty is performed to enlarge the narrowed opening and facilitate the flow of blood through them..

How is Valvulopasty done?

The procedure is performed in a cardiac catheterization laboratory and takes up to four hours. The patient is usually awake, but is given local anesthesia to make the area where the catheter is inserted numb. The site where the catheter will be inserted is prepared and anesthetized. The catheter that has a small deflated balloon at the tip is inserted through the skin in the groin area into a blood vessel, and then is threaded up to the opening of the narrowed heart valve. The balloon is inflated, which stretches the valve open. Once the valve is widened, the balloon-tipped catheter is removed..

You’ve also seen that much of the testing and surgery done today in response to heart disease is not only unnecessary but also downright perilous to the patient. Angioplasty and stents don’t “fix” patients. They’re the equivalent of cosmetic surgery for the arteries: they may make them look excellent, but they don’t do anything but cover up the problem. Heart bypass surgery is rarely the answer either. It doesn’t do anything to change the underlying issue that made the need for surgery in the first place.

But you’ve also seen how you can drastically reduce your odds of developing cardiovascular disease, cancer, diabetes, hypertension, and stroke, as well as other illnesses related to unhealthy diet and lifestyle.

The Ozner Ten-Step Prevention Plot:

l. Follow a Mediterranean Diet. The Mediterranean diet has been consumed by the peoples of the Mediterranean basin for thousands of years, and is the only diet that has ever received so much scientific scrutiny and validation: carefully documented scientific research conducted over the past six decades has conclusively proven that this diet drastically reduces your risk of cardiovascular disease. It’s also the basis for the diet I have successfully used in my preventive cardiology practice for over twenty years. It features vegetables, fruits, whole grains, legumes, nuts, red wine, minimal lean meat, extra virgin olive oil as the primary source of stout, plenty of fish, and ideally, no processed foods, refined carbohydrates, or trans fats.

2. Exercise Regularly. Studies show that exercise doesn’t have to be excessive, and it doesn’t even have to take place all at once, but one way or another, you need to get at least thirty minutes of it daily to maintain optimum cardiovascular health. Forget the excuses. It doesn’t require an expensive gym membership (although gym memberships are fine, if used) or an investment in bulky or expensive exercise equipment. A simple pair of comfortable walking shoes should be sufficient to start you on the road to more energy and better health.

3. Manage Your Stress. Relentless stress is toxic to the mind and body. Take time to invest in relaxation. Practice yoga or tai chi, take rage management classes, laugh it up with friends, practice meditation, or find a relaxing and engaging leisure activity. Do whatever it takes to catch your breath, cool down, and disconnect from the hustle and bustle of your busy life on a routine basis. The world won’t come to an end if you do. And if you don’t, you may well come to an abrupt end.

4. Take Command of Your Blood Pressure. Monitor and control your blood pressure. Follow a diet and lifestyle that keeps your blood vessels flexible, and your blood pressure below 120/80 mmHg.

5. Control Your Cholesterol. Monitor and control your cholesterol level. Follow a diet and lifestyle that keeps HDL cholesterol high and LDL cholesterol and triglycerides low.

6. Reduce Free Radicals and Oxidative Stress. There is a war going on in your body between free radicals and antioxidants. Fight free radicals, and the oxidative stress and disease that results, by giving your body’s natural antioxidants plenty of reinforcements in the form of fresh fruits and vegetables. Avoid toxins, including cigarette smoke, to keep free radical production as low as possible.

7. Avoid Chronic Inflammation. Control inflammation by following the anti-inflammatory Miami Mediterranean diet and consuming plenty of omega-3 fatty acids in the form of cold water fish, and certain vegetable and grain sources. Proper dental hygiene and regular dental cleaning and check-ups help to guard against a frequent cause of chronic inflammation, chronic periodontal disease. Treating other conditions that lead to chronic inflammation, such as chronic bronchitis, prostatitis, and arthritis, also helps to quell the fires of inflammation.

8. Prevent Metabolic Syndrome and Diabetes. Prevent and reverse metabolic syndrome and its frequent end result, type 2 diabetes, by following prudent diet and lifestyle recommendations.

9. Have an Annual Physical Exam with Comprehensive Lab. An ongoing relationship with your personal treating physician is vital. Have an annual physical exam and discuss prevention strategies with your doctor — even if you are healthy and feel well. In addition, have an advanced lipid profile done. Specifically; make sure your doctor tests for three things: particle number, hs-CRP, and LP(a). These laboratory tests provide greater insight into the specific cardiovascular disease risk factors that may impact your individual situation.

10. Avoid Unnecessary Diagnostics Tests and Procedures. Routine cardiovascular screening with CAT scans are not recommended for healthy people — they are expensive (and not covered by insurance), and they deliver a significant dose of ionizing radiation that may increase the risk of cancer. Nuclear stress tests also result in radiation exposure and can usually be replaced by a stress echo (ultrasound) study. Cardiac catheterization is not always as safe or simple a procedure as you may be led to believe. Since it often leads to simultaneous procedures, such as balloon angioplasty and/or stent placement, you may end up with far more than you bargained for. Don’t let yourself be talked into coronary bypass surgery unless it’s truly indicated. Seriously weigh the pros and cons before submitting to any of these procedures. And do not hesitate to seek a second opinion if your physician is unwilling to discuss risks, benefits, and alternatives.

You can prevent, halt, or even reverse heart disease. The choice to do so is up to you.
